It is necessary

- - copies of parents' passports;
- - certificate of change of surname, name, patronymic (if any);

Instructions
Step 1
Fill out the questionnaire for children on a computer in capital letters with Caps Lock enabled, without abbreviations, in pdf format. To do this, install the Adobe Acrobat Reader program on your computer. To fill out the front side of the application for obtaining a passport for a child, fill out columns 1 to 12.
Step 2
Write in the first line of item 1 of the questionnaire the full name of the child in capital letters, in the second line - “F. I. O did not change (a) ". In paragraph 2, indicate the date of birth of the child, for example, "August 19, 2009". In the 3rd column, write the gender of the child, in the 4th - the place of his birth, for example, “Mr. Pskov”, in the 5th - the place of registration of the father or mother (applicant).
Step 3
Go to step 6, indicating the child's citizenship "Russian Federation" on the top line. On the bottom line, if you do not have citizenship of another country, write “not available”. In column 7, enter the details of the birth certificate or passport (for children over 14 years old).
Step 4
Indicate in point 8 the purpose of obtaining a biometric passport. Most often they write here "for temporary trips abroad." If the purpose of obtaining a passport is to live in another country, name it, for example, “to live in England”.
Step 5
In paragraph 9, indicate the receipt of a passport "primary", "instead of a used one", "instead of a spoiled one" or "instead of a lost one." Step 6
If you have an old passport, indicate its details in column 12, otherwise leave the field blank. A child over 14 years old puts his signature on the front of the questionnaire.
Step 7
After filling out the front side of the application for a passport for a child, proceed to filling out the back side. Item 13, like item 1, consists of 2 lines. On line 1, indicate the full name of the child's legal representative. If the name and surname have changed, then in line 2 it is necessary to indicate the previous data of the applicant, the registry office and the city. Step 8
In paragraphs 14, 15 and 16, enter the date of birth, gender and place of birth of the applicant, respectively. In the next 17th column of the questionnaire for children, enter full information about your registration - postal code, city, street name, house, building, apartment numbers, and also a phone number. In paragraph 18, indicate the series and number of the passport, the date of its issue and by whom it was issued, the code of the unit. Leave the rest of the columns blank.
