Step 1
To return your maiden name, contact the registry office where the marriage was registered and write a statement in which you indicate: - last name, first name, patronymic, date and place of birth, citizenship, nationality, place of residence, marital status; - last name, first name, patronymic, date of birth of all children under the age of majority; - data of marriage or divorce certificates that were issued earlier; - data of birth certificates of children; - surname you want to take; - reason for changing the surname.
Step 2
In order for your application to be considered, provide the following documents: - your birth certificate; - certificate of divorce or death certificate of your spouse; - birth certificates of children under the age of majority.
Step 3
Employees of the registry office must consider your application within 30 calendar days. If there are valid reasons, this period can be increased to 2 months. In addition, if you submitted an application for a change of surname not to the registry office that issued the marriage certificate to you, but to another, then this period, according to the law, can be increased to 3 months. This is due to the fact that employees must first make a request to the registry office in which you registered the marriage, and only after receiving confirmation from them, they can begin to consider your application.
Step 4
After the expiration of the specified period, you will be issued a certificate of surname change. Your surname will also be changed on birth certificates for minor children.
Step 5
After receiving a certificate of surname change, contact the passport office at the place of registration to replace your general civil passport. On its basis, change all other documents: TIN, pension insurance certificate, medical policy, driver's license, foreign passport, and so on.
