Instructions
Step 1
Obtain a notarized permission from the biological father for the stepfather to adopt the child. It is drawn up by a notary in a standard form. If the father refuses to give such permission or to appear in court to confirm his consent, then the child will not be adopted. The only way in this case is to deprive him of his parental rights.
Step 2
Contact the guardianship authority and draw up a statement about the desire to adopt the wife's child. Please note that you must apply at the place of registration of the adoptive parent. However, during the process, you will also need to communicate with the guardianship authority at the actual place of residence and place of registration of the child. Adoption will be easier if both the child and his mother and stepfather live and are registered in one place.
Step 3
Order a certificate of no criminal record. You can get it through the Unified Portal of State and Municipal Services. It takes about a month and is valid for 6 months, so you can order it in advance.
Step 4
Go through a medical examination and receive a health report on the appropriate form for adoptive parents on form No. 164 / y-96, certified by the head physician. You will need to undergo a therapist, infectious disease specialist, dermatologist, venereologist, phthisiatrician, neuropathologist, oncologist, psychiatrist and narcologist, as well as pass a general urine and blood test, do fluorography and be tested for syphilis. The conclusion is valid for 3 months. If during this time you do not have time to adopt your wife's child, you will have to undergo a medical examination again.
Step 5
Take a salary certificate at the place of work in the form of 2-NDFL. The stepfather does not need to have income to ensure the subsistence level of the child, therefore this document does not actually affect the decision on adoption, but it may be required.
Step 6
Ask at work to draw up your description, which must be certified by the seal of the enterprise and the signature of the director.
Step 7
Prepare an extract from the house book and prepare documents for the apartment. They will be required to conduct a survey of your home to determine if it is suitable for your child.
Step 8
Write an autobiography. Write it in free form in chronological order. The document is not certified.
Step 9
Obtain a certificate of the child's health in the form of 160 / y from the polyclinic. A blank document can be obtained from the guardianship authority. It is he who is filled in. No other references are considered. After the child undergoes a medical examination, the document is signed by a commission of three doctors. Based on the certificate, the stepfather must write a statement that he is familiar with the state of the child's health.
Step 10
Submit a statement to the mother of the child in which she confirms her consent to the adoption.
