Summer vacation has come, and many children go on vacation to summer camps. This is a fun and carefree period that brings a lot of good impressions and pleasant memories. But on the eve of a fun holiday, parents will need to make sure that all the documents that the child needs to be admitted to the camp are prepared.

Step 1

To rest in a summer health camp within our country, a child will need:

- voucher and a receipt for its payment (if the voucher is paid);

- a child's birth certificate or his passport and a copy of these documents;

- medical policy and its copy;

- medical certificate in the form No. 079 / y (to be filled in by the local doctor) or No. 076 / y (if the child goes to a sanatorium-resort camp);

- a certificate of the epidemiological environment (issued by an epidemiologist at the clinic or at the local SES).

Step 2

Depending on the characteristics of the camp, you may also need:

- copies of parents' passports;

- certificate from a dermatologist;

- a certificate from a doctor authorizing a visit to the pool;

- permission of the pediatrician to engage in certain sports;

- a photo of a child 3x4.

The need to provide these documents is reported when the voucher is issued.

Step 3

If a child goes on vacation to a camp abroad, the list of documents will be different. You will need:

- the child's foreign passport;

- a voucher to the camp and a receipt for its payment;

- a notarized power of attorney for the accompanying person;

- visa (its registration is required only for some countries);

- medical insurance;

- fare payment (if it is not provided by the voucher);

- a medical certificate or a corresponding questionnaire (in some camps it will be necessary to provide a translation of a medical certificate).

Step 4

Additionally, for foreign camps you may need:

- questionnaires filled out for the child and parents (in English);

- copies of parents' passports;

- a characteristic from the school, a certificate of school attendance;

- birth certificate of the child;

- photographs of the child.
