After the decision to send the child to the camp is made, it is worth thinking about collecting the documents necessary for the trip. If, before boarding a train or plane, it turns out that some document is missing, the trip will be irrevocably ruined.
Instructions
Step 1
It is better to start filling out the documents a few weeks before the planned trip to the camp in Russia. If the camp is outside the country, it is better to take a few months to collect documents. The list of documents for going to the camp is usually standard, but there may be deviations from the rules, so it is worth inquiring in advance about the requirements for a children's health resort, from the tour operator or at the place where the voucher was purchased.
Step 2
Without fail, when leaving the camp in Russia, you will need a voucher, completely filled out, with signatures and seals, indicating the dates of arrival and departure; identity document of the child (birth certificate or passport); health insurance policy (card and paper version); certificate in the form 079U and certificate of contacts (issued no earlier than three days before departure).
Step 3
If the certificate is not taken at school or in kindergarten, then the parents will have to issue it on their own. To do this, you will need to bypass several doctors, pass the necessary tests, wait for their result and, in the end, visit the district pediatrician, who will sign and issue a completed certificate. All that remains is to put the necessary stamps in the registry.
Step 4
In addition to the package of documents, photocopies of parents' passports, one or more photographs of a certain format, doctor's permission to practice certain sports, etc. may be required. If you plan to cross the territories of other states on the way to the camp, you will need a notarized consent for the accompanying person.
Step 5
If a child goes to a camp outside of Russia, the package of documents will increase significantly. To leave, you will need a valid passport, visa, parental consent for the trip, certified by a notary, medical insurance, a certificate of the established form, completed questionnaires (in English), several photographs, as well as copies of the parents' Russian passports and the original of the child's birth certificate.
Step 6
In order not to be left without documents upon the return of the child, copies can be given for the trip, leaving the originals at home. As a rule, the administration of the camps is loyal to such requests from parents. Any camp at its discretion can request additional documents. You can get acquainted with the list of requirements on the official website of the camp, ask questions on the contact numbers of the camp administration, or clarify all the details of the trip with the tour operator. Parents, in turn, can attach a note to the package of documents describing the psychological characteristics of the child and the existing medical problems.
