Necessary

  • - birth certificate from the hospital
  • - coupon from the generic certificate
  • - the third part of the exchange card about the child's condition
  • - child's birth certificate
  • - permanent registration of the child
  • - compulsory medical insurance policy
  • - SNILS

Instructions

Step 1

When you are discharged from the hospital, you are given the first documents for the child. This is the third part of the exchange card, which contains all the information about the baby, and must be given to the visiting nurse when you visit you for the first time. This data helps pediatricians to improve the quality of care, as well as provide the necessary assistance to your child. You also receive a coupon of the birth certificate, which you provide to the children's clinic, where the baby will be monitored - you can choose any institution that you like. The maternity hospital also gives you a birth certificate stating that you are the mother.

Step 2

A birth certificate is valid for only a month, and during this time you need to make a birth certificate of the child. You can get it at the registry office at the registration of any of the parents. In the case of official registration of marriage, one of the spouses is sufficient. To do this, he needs to have with him both passports and their copies, a marriage registration certificate and a copy. You will receive a certificate immediately after submitting all documents. If your marriage is not registered, then both must be present and the man must agree to recognize the child, otherwise there will be a dash in the "father" column. In the registry office, you will also be given a certificate in form 25, which will be needed to receive a one-time allowance for the birth of a baby - it must be provided at the place of work or to the social protection fund within six months.

Step 3

Another necessary document for a child is a compulsory health insurance policy. It can be obtained from the insurance company chosen by the parent. To do this, you must provide there a passport and a child's birth certificate. Initially, a temporary one is issued, and about a month later - a permanent one, which does not have a validity period and makes it possible to receive free medical care in specialized state institutions, regardless of registration. A copy of the policy must be given to the children's clinic, where the child will be served.

Step 4

The child also needs permanent registration, which is done at the passport office. It should be borne in mind that you can register a child only with one of the parents, and the consent of the other owners of this housing is not required. To do this, you need to give the passports of both spouses and their copies, a marriage registration certificate and a copy, a child's birth certificate and a copy, and also write a statement about the desire to register the baby with you and another statement that the second spouse is not against it. Permanent registration is done within a week, after which all documents are returned.

Step 5

It is advisable to immediately get SNILS for the child, although this is not a mandatory document, but recently it has been required in many places, including when queuing up in the garden, to receive free meals, etc. It can be done at the pension fund at the place of residence, by giving there the passport of one of the parents and its copy, the child's birth certificate and its copy, and you also need to write a statement. Within a month you will be given a green plastic card.

Step 6

If you want to travel with a child outside the country, you must obtain a foreign passport for him. To do this, the birth certificate at the registry office must be stamped with citizenship - this can be done immediately upon receipt of the certificate, and then contact the passport office at the place of registration of the baby with all the documents and their copies.
