Your child has missed one day of school. To justify him, you, as parents, need to write a note to the homeroom teacher stating the specific reason for your child's absence from school.
Instructions
Step 1
Take a sheet of A4 format and in the upper right corner of the sheet indicate the following details of the addressee: position, name and number of the school, city (village) where the school is located, surname and initials of the head (Director of secondary school No. 1 in Velikie Luki, Simakov E. I.). Under the details of the addressee, write your surname and initials (from Petrova T. I.).
Step 2
Next, in the center of the sheet, indicate the name of the document (for example, "Application"). Then tell us what was the reason for your child's absence from school. It is necessary to start with the words: "In connection with …" or "My son Andrey Petrov was not present at the classes on March 10, 2012 in connection with …". It is advisable to indicate valid reasons, such as feeling unwell, leaving the city for family reasons, unforeseen visits to the dentist. You can also attach documents confirming what you wrote in the application, for example, tickets to another city or a certificate from the clinic.
Step 3
At the end of the note, do not forget to affix the date of its preparation and personal signature, followed by decoding.
Step 4
Give the note to your child to give to the homeroom teacher.
