All citizens of our country, including the smallest, have the right to medical care. By law, the child's parents can choose any health care institution they like. If you are moving to a new place of residence or for some reason decided to change one clinic to another, you need to attach the child to a new medical institution.
Instructions
Step 1
Do not take any action if your baby is just born. Newborns are automatically assigned to the local polyclinic. When discharged from the hospital, doctors will ask at what address the child will live. They will send the information themselves to the polyclinic serving your home, and the next day after you are discharged, your local doctor and nurse will visit you.
Step 2
Take the child's medical and vaccination card from the old clinic if you change your place of residence. Notify the district pediatrician about the move, inform the registry of your new address and sign in the journal that the child's medical documents have been received. Take them to the new clinic and give them to your doctor. The medical card will be assigned a number, this will mean that now the responsibility for the health of your child lies with this medical institution.
Step 3
Select the clinic where you want to see your child if the local clinic does not suit you. Collect the child's outpatient and vaccination card from the registry of the former medical institution. Put your signature in a special journal, which is located in the registry, that the child's medical documents are in your hands. Contact the registry of the selected polyclinic and inform that you want to attach the child for further observation. You will be asked to write an application addressed to the chief physician, after which the card will be numbered and a treating physician will be appointed.
