Step 2
Prepare the required documents. For a one-time benefit, prepare, in addition to the child's birth certificate, a certificate from the work of the second parent stating that he did not receive the benefit. If the second parent does not work, you can provide a work book with a letter of dismissal or a certificate from the educational institution where he is studying.
Step 3
To receive a lump sum and childcare allowance, write an application to the organization where the mother works. If she is not currently engaged in work, the application must be addressed to the father's place of work. The application can be either handwritten or typed on a computer.
Step 4
At the top of the application, indicate the name of the organization, the official. to whom you are addressing your appeal - the director of the enterprise, as well as your last name, first name and patronymic. Next, write the heading "Application". In the text itself, indicate what type of benefit you want to be assigned. Below, put the date of the document, your surname and initials, as well as your signature. Also write down in the paragraph entitled "Appendix" the list of documents to be provided - a child's birth certificate and a certificate from the work of the second parent.
