Entering kindergarten causes a lot of excitement on the part of the parents and the baby. The child enters a new world in which he finds himself alone with others without the presence of his parents. The period of adaptation to the kindergarten requires a lot of patience from adults. It is difficult for a kid to break away from people close to him, to get used to educators and other children. But all this is preceded by the time of collecting documents. This procedure will take most of the time, so it is important to plan your actions in advance. Knowledge of the documents necessary for admission to the kindergarten will allow you to correctly allocate your time.
Instructions
Step 1
The most important and difficult document to complete is a medical record. It is drawn up at the polyclinic at the place of residence. With this card, you will need to go through all specialist doctors: ophthalmologist, dentist, ENT, psychologist, surgeon, neurologist, cardiologist, orthopedist, pediatric gynecologist. After examination by specialists, the card is filled in by the pediatrician. You will need to pass urine tests, feces for an egg-leaf and a smear for enterobiasis (this test is taken a week before the start of visiting the kindergarten). At the end, the card is certified by the signature and seal of the head of the preschool department of the children's polyclinic. A medical card must be presented to the kindergarten when applying for admission. The medical record will need to attach copies of the birth certificate and medical policy.
Step 2
Help on the composition of the family. It testifies to the registration of the child in the region in which he goes to the kindergarten. If you have not yet managed to register the baby, then upon admission to kindergarten you will need to register him.
Step 3
Birth certificate of the child and its copy.
Step 4
A copy of the mother's passport and a questionnaire, which contains brief information about the parents (full name, date of birth, place of work and position, telephone numbers for communication).
Step 5
Documents or copies of them confirming the family's benefits. This can be a certificate from the social protection authorities on the recognition of a family as poor, a certificate of a mother with many children, or a certificate confirming other benefits.
Step 6
A voucher for sending a child to a kindergarten. In some regions, the referral is given to the kindergarten, and not to the parents' hands. After that, the kindergarten staff inform the parents about the need to come to the kindergarten for paperwork.
Step 7
After submitting all the documents described above, an application is written with a request to admit the child to kindergarten.
