The application itself for registration of the document can be filled out both in the selected insurance company with the direct participation of a specialist, or prepared at home. It indicates both the baby's data and the personal information of his legal parents or representatives. Today, busy dads and mothers can be replaced by their own grandparents by other relatives, who, with a simple written form of power of attorney, can come to the insurance company on behalf of the child's parents, taking all the above-mentioned papers and documents. It must also be said that by the time the temporary certificate is received, it is advisable to register the child, however, today insurance companies also practice indicating the official place of registration of one of the baby's parents.

At the very moment of signing the application, a so-called temporary certificate is handed over to the representatives, the official form of which guarantees the receipt of a centrally issued indefinite policy and the opportunity to receive a full list of all medical services guaranteed by the state's free health insurance system. Both the expiration date of the temporary certificate and the official notification sent by the selected insurance company will tell you about the time of receipt of the permanent policy.
