Compulsory health insurance policy (MHI) is a document on the basis of which the insured person is entitled to receive free medical care. Since 2011, you can get it for a child in any health insurance company included in the register of medical insurance organizations, which are posted by the territorial CHI fund on its official website or published in other ways.
Necessary
- - copies of the passport;
- - birth certificate of a child;
- - registration certificate.
Instructions
Step 1
According to the order of the Ministry of Health of February 2011 - the insured person has the right to choose a health insurance organization. Compulsory insurance of the child is carried out by the one in which his mother or other legal representative is insured.
First, select the organization in which you plan to get an OMI policy. Each territorial district has a rating of companies providing compulsory health insurance services.
Step 2
Insurance will be carried out on the basis of an application and copies of documents. Prepare a certified birth certificate for the child and a copy of the passport of the legal representative (mother or father). In the application, indicate the child's full name, gender, date of birth, place of actual residence, citizenship, place and date of registration.
In the application, also indicate the same data of the legal representative and the attitude towards the insured person (child). Make a written (or printed) statement and submit it directly to the insurer. Or fill out an application in electronic form and post it on the official website of the territorial compulsory health insurance fund. After registration, a notification with further instructions will be sent to the email address you specified in the application.
Step 3
Currently, there are electronic insurance policies in a number of regions. They are a plastic card with an electronic carrier. In other regions, the compulsory medical insurance policy exists in paper form. It is issued for an unlimited period of time. On the basis of his, the child has the right to receive free medical services within the limits established by the decision of the commission for the development of the territorial CHI program.
