Step 1
Generic certificate. In the maternity hospital, the second coupon is taken off the certificate. It is the basis for transferring funds to the institution for providing medical care to a woman in labor. At checkout, the certificate is returned. It contains two coupons to pay for the services of a district pediatrician. The first coupon pays for services for the first half of the year, the second for the next six months.
Step 2
Discharging from maternity hospital. It contains full information about the course of delivery, about the procedures performed, data on the weight and height of the child, the results of the tests and analyzes performed, as well as about the vaccinations made. You will later give the extract to the district pediatrician. This document will be kept on the child's medical record.
Step 3
Exchange card. In the maternity hospital, information about the course of childbirth and about the child is entered into it. After a week, you will need to take it to the gynecologist with whom you were registered. This card is needed for the reporting of antenatal clinics. At the same time, the gynecologist conducts an examination to exclude possible complications after childbirth.
Step 4
Birth certificate of a child. She certifies the fact of the birth of the baby. It contains the date of birth, surname, name and patronymic of the child and the name of the obstetrician who delivered the baby. On the basis of the certificate, a birth certificate is issued in the registry office. And also it is the basis for the issuance of a lump sum. The validity of the certificate is limited to one month.
Step 5
Child's vaccination certificate, which contains information about the vaccinations given at the maternity hospital. This document is optional for the extract. In some regions, the vaccination certificate is later brought by the district nurse.
Step 6
Child's birth certificate. This is not a mandatory document for discharge, but it may be required in some maternity hospitals. The number of the certificate and the name of the child are entered in the statement.
