The arrival of a child is a very joyful event in the family, but it is overshadowed by the fact that parents are faced with paperwork. A huge number of documents, certificates from various places - some for the clinic, others for children's consultation, and others for benefits. And they are all important in their own way. Therefore, mothers and fathers need to clearly understand which institutions need certain documents.

Step 1
The childbirth process is the most difficult stage in the life of every woman. But we must also not forget about the importance of documents that must be issued to each mother for her and her child. This is, firstly, the rest of the exchange card, which is issued in the antenatal clinic, sheets 2 and 3, where the second sheet contains information about the nature of childbirth, the state of the woman in labor, and in the third - information about the child's condition, physiological parameters (weight, growth).
Secondly, coupons No. 3-1 and No. 3-2 of the generic certificate for payment to health care institutions for services for the first and second six months of dispensary observation, respectively.
Thirdly, a certificate of the birth of a child from a maternity hospital or a medical birth certificate. It is valid for a month, must be completed by the midwife who delivered the baby, and contain information about the child's gender, time and date of birth. The birth certificate of the child is certified by the signature of the midwife and the seal of the maternity hospital.
Step 2
For the issuance of a birth certificate by the registry office, it is necessary to provide a certificate of the birth of a child from a maternity hospital, a mother's and father's passport and a marriage certificate (if any), as well as a statement from one of the parents. Together with the birth certificate, a certificate in form No. 24 is issued to receive a one-time allowance for the birth of a child, which is valid for six months. After this time, the right to receive federal benefits in accordance with Article 17.2 of the Federal Law of 1995-19-05 No. 81-FZ is not retained. It should be borne in mind that this allowance is issued at the place of study, work of one of the child's parents. To receive the allowance, the following documents are required: an application for the appointment of an allowance, a birth certificate in form No. 24 of the registry office, a certificate from the place of work, study of the other parent, that such an allowance was not assigned.
